In order to access the Patient Portal, you must verify information on your account.  The information that will need to be verified is based on the information on file for the Guarantor assigned to each child.  If you are listed as the guarantor for multiple children then your 'one account' will be able to access information for each of these children that you accept and verify information for.  (You may not be required to do a verification for each child but, an invitation must be sent for each child in order for the link to be completed) 

The information that will be requested to be verified is First Name, Last Name and Date of Birth (of the guarantor) and Zip Code.  The system will only accept the 5 digit Zip code and not a 5+4 digit Zip Code.

The patient portal will allow you the ability to check information on past visits, download and securely send medical records to another secure location, send messages to staff, update your account information, request prescriptions, check your current account balance and make a payment with out requiring information from your statement.

FYI:  each invitation link is only valid for 72 hours.  If it has been more than 72 hours from the time that your invitation was sent to you please call our office to have the invitation resent.
